Cushing's Syndrome

Causes

  • Ectopic Cushings

    • ACTH producing tumor

    • Pituitary produced too much ACTH

  • Adrenal gland tumor

Indications

  • Increased chloride (> 106 mmol/L)

  • 0800 Cortisol (>635nmol/L, >23mcg/dL)

  • 1600 Cortisol (>413nmol/L, >53mcg/dL)

  • Fasting glucose (>6.1 mmol/L, >110mg/dL)

  • Insulin (>186pmol/L, >26mcU/mL)

  • Potassium (<3.5 mmol/L, <3.5mEq/L)

  • Thyroxine, total ( <64nmol/L, <5 mcg/dL)

  • Thyroxine, free ( <13pmol/L, <1.0 ng/dl)

Supplementary notes

  • Ovarian and Adrenal tumors can present w/ elevated T in Women

  • Pituitary insufficiency (hypopituitism overlap with Mom's symptoms)
